I agree in some situations PH can be used effectively for real-time work.
In my experience the effort and portability for such needs are better server
by C, C++ or Java. These tools have built in event triggers and don't rely
on OS based features such as mailboxes.
Windows based machines, and other not-as-good-as-VMS systems do not have the
mailbox feature. COM objects can be used quite effectively for inter-app
messaging but PH is ignorant of them without using an external procedure
written in a 3GL. The latest (though not efficient) messaging methods use
XML services. PH is not XML aware to my knowledge.
One of the real beautiful things about PH is that you can write an app that
runs on almost any OS/DB combination, as long as you avoid features such as
VMS mailboxes. :)

Quick that runs as a detached process that works beautifully (using VMS
mailboxes, talking to a Burr Brown TM9000) for our labeling/barcoding
system, which spits out barcoded labels and records serial number data on
demand from many production lines running simulateously.
