having problems deleting from ksam in qtp

Not sure about the KSAM problem. Have you verified that the data is still
there using QUIZ? KSAM (at least KSAM Classic) didn't actually physically
delete the records, it just marked them for deletion. You'd have to
occasionally recreate the file to get rid of them.

As for the second, try ON CALCULATION ERRORS REPORT on the REQUEST
statement. A data conversion error falls into this area. The ON ERRORS on
the OUTPUT statement deals specifically with file type errors.
