"Data has changed since you originally found it. Can't Change or Delete."

Has anyone seen this error message from PowerHouse web before?
Recently just a few users out of hundreds are receiving it on submitting a simple webpage. I've seen it on a regular terminal screen but not PH Web. It was due to changes in the post update procedure, but that's not happening here. The page hasn't changed in many years, but was recently ported and recompiled on an Itanium Integrity server.

There's nothing in the documentation. It's been suggested it could be a faulty proxy server. Anyone have experience of this and if so, what caused it and how was it solved? All suggestions/info appreciated.

OpenVMS V8.4 for Itanium
Oracle Rdb V7.3-130
