Hi All,
many thanks for all the ideas. 
Through using some of the ideas you sent I've now discovered what was causing the problem. We have 2 databases, the first is an Invoice Query database, the second an Invoice Adjustment database. If there is a problem with an invoice, a query is raised against the despatch ticket and then an adjustment is raised against this query. When the adjustment is completed the QTP routine I was having the problem with takes a list of all completed adjustments and jumps across to the query database and resolves the appropriate query. What I discovered through further investigation, is that the business previously only had 1 query per despatch ticket, they are now creating multiple queries on the one despatch, so what I thought was a unique record was not, hence the 'Record has changed since you found it' error. I have now added an extra file linkage which includes and absolute unique field which is used in the 'select' and an 'output at' and the problem seems to have been resolved.
I was obviously a bit to keen with the original post, because if I'd waited an extra couple of hours I would have discovered what the users were actually doing.
