Upgrade from 7.10.G1 to 8.40D

Hi all
As part of an overall systems upgrade from:
VMS 7.3-1 and OracleRDB 7.1-03
to
VMS 7.3-2 and OracleRDB 7.1-04
we are upgrading Powerhouse from 7.10.G1 to 8.40D.
We've discovered a few things that didn't work as expected.
Under 8.40D, we now have only four .phd files, whereas under 7.10.G1 we had FIVE .phd files. The dictionary was converted using the PHDCONV utility. Is this correct, does the new version have only four .phd files, or are we missing something?
When building our 8.40D QKGO file, we used the 'copy' function within QKGOMAINT to copy (and convert) from a 7.10.G1 QKGO file. According to the manual, it should say "Convert an existing 7.10 QKGO file [Y/N]", but the only question it asks is the second one we expected, ie "Copy old TICs?".
Using the dictionary and qkgo file created in the preceding steps, our application will start up, showing us the highest level menu screen. Attempting to go to a subsequent screen results in a "fatal error 604" message. Does anyone know what this error means?
Our QKGO file doesn't appear to have transferred the DFK definitions. If we enter them manually, and then try to view the data we've just entered, QKGOMAINT tells us that there is no data. If we believe this message and try and re-enter the definition, we get an error saying that key and data definitions already exist for this key.
MATCHPATTERN appears to be giving us results inconsistent with what we were getting in 7.10.G1. Is this a known problem, and if so, is there a workaround or a fix for it?
