Thank you to everyone who responded.  I wound up with:

SET REPORT DEVICE PRINTER NAME fj211l
PAGE HEADING " " SKIP 5
SET PAGE WIDTH 40
SET PAGE LENGTH 58
SET PAGE IMAGES 2
SET FORMFEED

The primary source of amazement was that "page heading " " skip 5" and "page
heading skip 6" work so very differently.  I did have a professor in
university make the statement that the only reason we were in computer
science was that our mothers had dropped us on our heads as small babies!
In instances like this I am inclined to support his position.
