We had a situation which gave the same error message a few months ago.  At
the time, we were running Powerhouse 710F2 on an Alpha system under OpenVMS
7.1, and experiencing a rapid increase in the number of users. 

Our users start the Powerhouse application via a command procedure, which
uses 

	$   quick auto=smart$bin:srs.qkg entryrecall broadcast=default

The error message occurred when we were running with a number of reserved
licenses.  With the help of Cognos Sydney staff, we found that the problem
occurred when the number of reserved licenses plus the number of
non-reserved licenses in use was equal to the license limit, and another
user attempted to run the Powerhouse application. This would cause the
access violation message to be displayed when the first (compiled QUICK)
screen attempted to run, and the user was returned to the DCL prompt.
 
We found that we could confirm that there were not enough licenses for a
particular user by running QUIZ from the DCL command line after the access
violation message was displayed.
