Banner Page using Quiz

Try using the inital page statement.  This allows the first page of a report
to be different than all the other pages in a report.  As far as the users
name goes, will it be different users this is going to all the time or the
same user.  If it's the same user, you can hardcode the name.  If it's a
different user running the report each time, you'll have to define the users
name and use the defined field on the initial page.
