Between Quiz 4.00 and Quiz 5.01, Cognos changed the assumed file for an   
unqualified item from the last file in the access statement containing   
the item to the first file. This caught a lot of lazy programmers (the   
kind who never qualify anything) and resulted in report programs which no   
longer worked.
